Components

  2. Table of Contents

  3. Demographic tables/school information*

  4. Timeline/Mini-Unit Outline (table with dates)*

  5. Pretest* (test & key)

  • Data (in table format) – labeled correctly

*Graphs/Tables (label each graph/table) - labeled correctly:  Figure 1.  Classroom demographics

*Interpret graphs/tables in short paragraph format

Formative Assessment Rubric & Formative Assessment

Lesson #1

  • Lesson Plan (use music lesson plan template)

  • Rubric (approach, meets expectation, and exceeds expectation)

  • Data/Evidence/Artifact (documentation of three students’ work samples – either their work or a photo, labeled correctly)

  • Brief explanation of each artifact

    • How did this assessment improve learning?

    • What will you do in subsequent lessons to extend the learning
